THE SEASON IS NEARLY UPON US!
Ø
All athletes should have their parent/guardian check them in at their
Team Table the first day of summer camp/practice.
Ø
At check-in, the parent/guardian will complete and/or confirm:
Payment in Full and that we are holding a CURRENT Physical Form.
Ø
Please be sure your child’s physical is current for the duration of
their season.
Ø
If your child’s last physical was BEFORE AUGUST 13, 2007 we will need an
updated form.
Ø
If your child’s annual physical expires DURING the season (ANYTIME AFTER
August 13,2007), then at check-in we will either need a copy of your
CURRENT physical OR the date of the upcoming physical appointment.
Ø
If your child’s doctor does NOT require an ANNUAL physical we simply
need a form, signed by your physician, that your child is
cleared for play for the ’08 season. (Having your doctor sign the
physical form found on this web page will do the trick!)
The safety of your child is a primary concern.
We will need a Dr.’s signature on a current physical form in order for
your child to participate.
